Student Forms

Student Forms is used to submit documents requested to complete the verification process, or to request SAP appeals, Special Circumstancres reviews, and Cost Of Attendence appeals. Submit the required documents as soon as possible to avoid delays in processing financial aid. 

How to Create a Student Forms account:

  1.  Visit sjsu.studentforms.com
  2.  Enter student information exactly as it appears on FAFSA or CADAA. For CADAA applicants, use your CSAC ID number instead of a social security number.

IMPORTANT:

  • The information submitted must be exactly as it appears on your financial aid application.
  • CADAA applicants will use their CSAC ID number as a social security number.
