You can refer to this page for all Graduate forms and instructions on how to submit your documents. Links to other SJSU offices and their forms are also available here. For the fastest processing time, follow the steps outlined on your form and attach all supporting information adequately.

Note: The minimum processing time is two days for all forms submitted.

Change of Classification in Graduate Program

Submission Instructions

  1. Click on "Change of Classification in Graduate Program" to open the form
  2. On the form, enter Dr. Thuy Le and his email address thuy.le@sjsu.edu as the advisor/committee name and email address
  3. Attached unofficial transcript and admission acceptance letter with the form and submit
Application for Curricular Practical Training (CPT) & Enrollment in EE 298i

Submission Instructions

CPT information is on the ISSS website. Submit your application through iSpartan.

In the iSpartan eForm, enter Dr. Thuy T. Le as the Department Advisor and electrical-engineering-group@sjsu.edu as the email address to route the request for processing.

Once your CPT is approved, you will receive a permission number via the additional note section of the CPT application. Use this permission number to add yourself to the one-unit EE298i class. Note that the units earned in Internship classes do not count toward the units or courses required to earn your degree.

If your advisor notifies you via email of missing information or errors, you must cancel your CPT application, correct the application and resubmit the application on iSpartan again.

If you are enrolling in EE 298i after the add deadline, use the Late Add petition form and registrar's office will add your internship course once approved. Your advisor will notify you of the need for a late add form in the "Additional Note" section as well.

If you do not receive approval after 1 (one) week from the submission date, Please contact Ms. Marta Ramirez-Rodenas at marta.ramirez-rodenas@sjsu.edu or meet her at the drop-in hours. For other questions about CPT, please contact ISSS.

Email the petition together with an unofficial transcript and current course schedule to Dr. Thuy Le at thuy.le@sjsu.edu.

Submission Instructions

If your employer requires a letter from the department to verify the classes taken and skills learned during master’s work for immigration processes, please follow the information below:

To receive a letter from the department citing the skills acquired during your master’s degree work in EE and signed by a department official on department letterhead, we ask that you craft the letter yourself using our template.

We have included some sample skills in this template; please customize your letter to fit your classes and skills. Once completed, email the letter along with your unofficial transcripts showing receipt of your degree to Dr. Thuy Le at thuy.le@sjsu.edu.

We will only provide one letter to each student. Please do not ask for a letter unless your employer requires it.
