Vision Benefits

Plans

There are two options for vision benefits; one paid for by the CSU, one paid for partially by the employee. Highlights of each plan are below. View the VSP Benefit Summary [pdf] for more information.

CSU Vision Plan - Basic
  • All eligible employees and their eligible dependents are automatically enrolled in this plan
  • Administered by Vision Service Plan (VSP)​ (requires SJSUOne/CSU login)
  • Premiums paid by the CSU
  • Visit any licensed ophthalmologist, optometrist or dispensing optician
  • Find an Advantage Network provider by logging into the VSP Member Portal or calling (800) 877-7195
  • Using an in-network provider will reduce your costs

Includes:

  • One comprehensive eye exam every calendar year
  • One pair of lenses every other calendar year (or calendar year if your prescription changes) and one frame every other calendar year
  • Contact lenses every other calendar year when contact lenses are provided in lieu of all other lens and frame benefits
  • Basic VSP Evidence of Coverage [pdf]
The VSP Premier plan
  •  All eligible employees may participate in the CSU VSP Premier Plan
  • Employee pays a small monthly cost share fee; additional cost will be deducted directly from your paycheck
  • Enables you to get a higher allowance for frames and contacts, fully covered standard progressive lenses, and more discounts
  • Enroll through VSP directly by calling (800) 400-4569. 
  • You may not enroll in both the Basic and Premier Vision coverage at the same time
  • Dependents you wish to cover must also be enrolled into the Premier Plan coverage; they cannot stay in the Basic Vision Plan

  • One comprehensive eye exam every calendar year
  • One pair of lenses/frames every calendar year
  • Contact lenses every calendar year when contact lenses are provided in lieu of all other lens and frame benefits
  • Extra Savings on Retinal Screening
  • Premier VSP Evidence of Coverage [pdf]
  • Claim form is not required when using standard in-network benefits.
  • Services provided by a non-VSP provider must be paid in full by the employee.

Accessing and Using Your Benefits

VSP Online Access

Register through VSP to view your benefits online. You will create a User ID and password during the registration process.

Claims – In-Network Benefits

A claim form is not required when using standard in-network benefits.
